Step One: Fill out the Facility Use Request Form

Step Two: Your Request will be evaluated

Step Three: We will contact you to let you know if your request is approved or not.

Facility Use Requirements:

Contact person takes responsibility for all persons attending the event.
Contact person is responsible for replacing any damage done to the building, grounds, equipment, and any church supplies.
Contact person is responsible for cleaning all areas used during the event this includes the common areas; hallways and both set of bathrooms.

You will need someone (Key Holder) to unlock the door. Do you know who that is? Put the Key Holders name here. (Required)

If you don't have someone listed we will get you the list of those who have a Key. The event can not be approved until you find your Key Holder.

If you need to use the Sanctuary do you need to use the Sound system or display anything on the TV’s?
