Facility Use Request Form - Process
Step One: Fill out the Facility Use Request Form
Step Two: Your Request will be evaluated
Step Three: We will contact you to let you know if your request is approved or not.
Facility Use Requirements:
- Contact person takes responsibility for all persons attending the event.
- Contact person is responsible for replacing any damage done to the building, grounds, equipment, and any church supplies.
- Contact person is responsible for cleaning all areas used during the event this includes the common areas; hallways and both set of bathrooms.
